Assante Wealth Management: physician signing bonus for new attendings

If you've just received a physician signing bonus, Assante Wealth Management suggests keeping it in cash until both the clawback period and the tax bill for that year are behind you.

The main exception is any slice of the bonus you can no longer be asked to repay and that has already been fully taxed. Only that slice should go to debt, a 529 or investments; everything else waits for the contract's step-down dates.

The repayment section of an offer letter is usually what turns a welcome deposit into a harder question. A $50,000 bonus can arrive as far less after payroll withholding, while the contract may still demand the full $50,000 if you leave early.

In first meetings, physicians at Assante Wealth Management keep raising the same two worries: “Can I pay down loans now?” and “What if this job isn’t right?” The safest answer starts with money you can reach on a bad day, not with a long list of accounts.

“It’s mine once it’s deposited”

The usual shorthand is, “Stay two years or pay it back.” The repayment clause is often narrower and tougher: full repayment in months one through 12, half in months 13 through 24, then no repayment after that. Some agreements prorate by month; others use annual step-downs. Read the repayment section of your offer letter before assigning even one dollar.

Cash in a savings account or Treasury bills can be used again any day. A loan payment, 529 contribution, or kitchen renovation cannot become cash again when a clawback notice arrives. Investments are saleable, but their value may be down on the day you need the money. Investments can lose value, and selling during a rough market may return less than you put in.

The practical holding period usually runs through the final clawback date and the following April 15, when the bonus year’s return is filed. That can be more than 24 months. Some contracts excuse repayment if the employer ends the job without cause or if disability intervenes; some do not. Kwame should check that one line first.

The offer letter sets the clock

Keep the bonus reserve where it can be reached quickly until the last clawback date and the related tax return are behind you.

“Payroll already took care of the taxes”

Payroll withholding on a bonus is a payroll calculation, not your final federal tax bill. Your W-2 arrives by January 31, but the actual answer arrives with the tax return. Assante Wealth Management would put the possible gap beside the clawback amount before discussing a loan payment or investment purchase.

Hypothetical: Kwame, 52, and Colleen, 50, sell their three-doctor practice in July. Kwame takes a hospital position with a $50,000 signing bonus, while Colleen leaves the front office to help their youngest prepare for college in two years. His contract calls for full repayment in months one through 12 and half repayment in months 13 through 24. Payroll withholds $11,000 for federal tax and $4,000 for other taxes, so $35,000 reaches their checking account: $50,000 − $11,000 − $4,000 = $35,000. Their ordinary income for 2026 is $825,000. After the IRS standard deduction of $32,200 for married couples filing jointly, taxable ordinary income is $792,800: $825,000 − $32,200 = $792,800. That exceeds the 2026 IRS threshold of $768,700 for the 37 percent top federal income tax rate for married couples filing jointly. At a simplified 37 percent rate, the $50,000 bonus produces $18,500 of federal tax: $50,000 × 37 percent = $18,500. Subtract the $11,000 withheld, and the federal gap is $7,500: $18,500 − $11,000 = $7,500.

Kwame’s practice profit, hospital salary, noncompete payment, and equipment depreciation recapture all belong in the ordinary-income calculation. In this illustration, the capital-gain portion of the practice sale is calculated separately and is not included in the $825,000 ordinary-income total. The practice-sale tax page covers that separate math.

Before the review, they planned to send the $35,000 to their variable-rate home equity line. Afterward, all $35,000 stays in cash through month 12. After April and month 12, they pay $7,500 to the IRS, keep $25,000 for the remaining clawback, and have $2,500 free: $35,000 − $7,500 − $25,000 = $2,500. Their full exposure is larger than the bonus cash, though. Leaving in month 10 could create a $50,000 repayment demand, so they need to know where the other $15,000 would come from.

A calculator on this site can help you run the same subtraction with your own bonus, withholding, and tax estimate. Assante Wealth Management would rather keep the reserve boring for two years, because a rushed sale or new loan is expensive when a job change lands at the wrong time.

The gross amount still matters

The two small mistakes that compound

Treating the $35,000 deposit as spendable and skipping the January estimate can create a $57,500 cash demand if Kwame leaves in month 10: $50,000 to the employer plus $7,500 to the IRS, before any underpayment penalty.

“Paying off a loan with it is the safe move”

Loan payoff feels safe, but it is the least reversible use of bonus money because the lender will not hand the payment back when the hospital requests $50,000. A borrower pursuing Public Service Loan Forgiveness gains nothing from prepaying debt that the program may forgive; the student loan planning page covers that decision. A few months of interest saved may cost the cash you need if the job, your health, or the contract goes wrong. Someone certain to stay may accept that trade-off, but a physician in a new city or a new employer’s first year usually should not.

How should I time the bonus deadlines over two years?

Keep cash equal to the larger of the gross clawback still owed or the amount you received, then add the estimated tax gap through April 15. For a $50,000 bonus with a $7,500 federal gap, that means identifying up to $57,500 of available cash in year one, even though only $35,000 hit checking.

The calendar tracks the $35,000 bonus deposit, not the extra $15,000 Kwame would need for a full first-year repayment. The cash requirement drops at April 15 and again at the month-12 step-down. The last dollars are not free until month 25.

Put the calendar beside the contract

Keep the proof of payment

For prior-year adjusted gross income above $150,000, the IRS federal safe harbor generally requires 110 percent of last year’s tax through withholding or estimated payments.

A one-page bonus plan from Assante Wealth Management

Assante Wealth Management can put the reserve, step-down dates, and April tax gap on one page you can explain to your spouse in five minutes. Before suggesting that freed-up money move to debt, college savings, or investment management for doctors, we check what the clawback still permits.

To start, send us a short note through the form on this page. Assante Wealth Management lists the full cost on paper before you sign anything.

A bonus is not spendable cash until the clawback and tax clock both run out.

Your questions about physician signing bonus, answered

What happens if I get sick and can’t finish the service period on my signing bonus?

Read the disability and termination language in the offer letter. Some physician contracts waive all or part of repayment after disability, while others do not. Keep the bonus reserve intact until you know the contract’s result. If the wording is unclear, have an employment lawyer or CPA review the clause before you rely on an exception.

Can I get back the tax I paid if I have to repay the bonus in a later year?

A later repayment can create a federal tax deduction or credit under IRS claim-of-right rules, depending on the amount and facts. Keep the offer letter, bonus pay stub, W-2, repayment notice, and proof of repayment with your paperwork. A CPA can compare the available approaches for the repayment year.

How long should a signing bonus stay in cash before I invest it?

Keep it in cash through the final clawback date and until the tax amount for the bonus year is settled. With a 24-month clawback and a tax return due the following April, that may mean more than two years. Money above the remaining repayment reserve and tax gap can be considered earlier.

Does a signing bonus count as pay for my 403(b) contribution?

A signing bonus may count as eligible compensation, but the hospital plan controls how and when you can defer it. Ask payroll before the bonus is paid. For 2026, the most an employee can defer into a 403(b) is $24,500, plus an $8,000 catch-up at age 50 or older, subject to the plan’s terms.

Is a bonus paid in January better than one paid in December of my first attending year?

January can give you more time to plan estimated taxes and may place the income in a lower-income calendar year. December may be better if a contract start date, retirement-plan deferral, or practice-sale income points the other way. Compare both years’ ordinary income, withholding, and clawback wording before choosing a payment date.
